Eye Glasses:     Triangle Vision Optometry

Eye Exams:       Triangle Vision Optometry

Procedure for Vision Assistance for Area School Children:

The Kernersville Lions Club is able to provide a limited amount of funding for eye exams and eyeglasses for Kernersville-area students with demonstrated vision deficiencies who also face severe financial hardship. The Lions have established partnerships with Triangle Vision Optometry in Kernersville to provide eye exams and basic eyeglasses as needed. We do NOT provide 'no line bi-focals or prescription sunglasses. These services and funds are reserved for children and families who have vision deficiencies (near-sightedness or other) that limit school function and who are unable to afford proper eye care.

The Kernersville Lions will rely on the School Nurse/Social Worker to identify cases, submit requests for funding, and ensure that children actually receive the care. The Lions have established following procedure for school social workers to request care:

The School Nurse/Social Worker will email Wayne Henderson (KvilleLionsSecretary@gmail.com) of the Kernersville Lions Club with a request for assistance, listing the child's name, home address, age, school, and need for exam, glasses or both, along with a very brief description of the child's financial need. School Nurse/Social Workers are encouraged to copy the school principal on the request. This request for assistance must be received PRIOR to contacting the medical professional.

The Lions Club expects the School Nurse/Social Worker to qualify students based on medical and financial needs. Lions Club will evaluate the request, approve (or reject), and reply to the School Nurse/Social Worker with a copy to Triangle Vision Optometry and Lions treasurer.

The School Nurse/Social Worker will, in concert with the parent or guardian, make the appointment with Triangle Vision Optometry, identifying the patient as supported by the Lions Club, and ensure that the child gets to appropriate doctor's office.

Eye professional will invoice the Lions Club for the exam and/or glasses per our agreement.
