SMITHSBURG LIONS CLUB

ANNUAL SCHOLARSHIP APPLICATION

YEAR 2011

PURPOSE

To encourage the positive development of young minds for the good of the nation through their association with Lions Clubs and their communities.

ELIGBILITY

Any graduating student in the Smithsburg High School district. The applicant must plan to enroll as a full-time college student.

REQUIREMENT FOR COMPETITION

Must have demonstrated citizen qualities through involvement in various extracurricular activities and/or community services.

TERMS OF AWARD

No monies will be paid directly to recipient except as stated herein. A check will be forwarded to the institution of the recipient's choice upon proper notification from the enrolling institution to the Secretary of the Smithsburg Lions Club indicating that the recipient is matriculated.

Applications must be submitted to the CHAIRMAN -Scholarship Committee. Mail applications to P. 0. Box 745, Smithsburg, MD 21783-0745, no later than April 15, 2011. Applications may be procured in the school's Guidance Office or may be requested by mail. Notification of the award recipients(s) will be made by the Scholarship Committee in writing to the applicant(s), no later than May 15, 2011.

The recipient will be expected to attend the meeting of the Smithsburg Lions Club on the fourth Wednesday in May, 2011 at 6:30 p.m. at the Mountain Gate Restaurant in Waynesboro, Pennsylvania to be awarded their $750.00 scholarship.
