Eyeglass & Hearing Aid Assistance 

Providing eyeglasses and/or hearing aid assistance is just one of the services our club offers to those community members in need. As long as the applicant meets the following criteria, he/she may be eligible to take advantage of this low cost to free service (dependent on applicants status).

How to apply to receive aid for eyeglasses or hearing: 

• Ensure that the applicant resides within the geographical region served by the Tacoma Southeast Lions Club (if applicant does not reside within the Tacoma Southeast Region; please feel free to contact us so that we may direct you to the proper club for your region).
• Fill out the application in it's entirety and to the best of your knowledge for full consideration.
• Provide the following documents to accompany your application upon return (only if applicable): 
  • Verification of Income (i.e. pay stub(s), bank statement(s), SSI Letter, etc.) 
  • Bank Statement(s) 
  • SSI Verification Documents 
  • Housing Documents
• It may take several weeks for this request to be approved or denied; the applicant will be contacted via the contact information in his/her application with status and instructions. 
• Be sure to forward all completed applications with corresponding verification documents to the Tacoma SE Lions Club via our Eyeglass and Hearing Aid Coordinator: Kim Riley, 6832 S Puget Sound Ave, Tacoma, WA 98409

Confidentiality Agreement

The information you have provided is used to determine the applicants' eligibility for assistance. The information may also be used to compile statistics that would further assist this progam and programs like these in the administration of the Lions Sight & Hearing. Personal information provided here will be treated in a strictly confidential manner in accordance with Washington State Laws and not be shared with other organizations unless given consent by the applicant or applicants advocate.
