Suddenly the attendant looks over at the front door, and there he sees a green sticker that will lead him to the answers he needs right in the fridge. Lion Kevin Mitchell has been working very hard on getting these bottles out to the public, which are free and will help safe your life. The form is easy to fill out with all your medication information, a picture of yourself, and any other information you want the EMS to know about you. Even your blood type.
