GOLF TEAM REGISTRATION FORM:

Come join us for eighteen challenging holes of golf, a burger or brat dinner with all the fixings, and various prizes.  The fun starts with a 9:00 AM tee-time.   A golf cart is provided for every two golfers.     

The total cost per golfer is $65.00 for the entire day of fun!
